Nu-Tek Levator Elite EMG ETS STIM background

Levator Elite is a single channel EMG Biofeedback with STIM and ETS function:

  1. EMG (electromyography) measures muscle activity and provides valuable biofeedback on muscle strength status.
  2. ETS (EMG Triggered Stimulation) is a combination of EMG and Stimulation during which EMG serves as a triggering threshold to commence stimulation. ETS facilitates rehabilitation of flaccid muscles, improves pelvic floor function and combats incontinence.
  3. STIM (stimulation - neuromuscular stimulation) improves blood flow and capillary bed density and strengthens muscle fibres.

Nu-Tek Levator Elite in EMG mode

In the EMG mode, the Levator Elite has 2 functions:

  1. Assessment
  2. Training

There is 1 pre-set (automatic) EMG program for assessment, enabling the user or the clinician to view their progress on the LCD screen. For the users, there is a simple bar graph display of muscle strength based on Muscle Strength Grading Scale 1 – 6 for Pelvic Floor Muscles, which helps the users to meet their therapeutic targets. For the clinicians, the Levator Elite displays the EMG Biofeedback measurements in a waveform format for clear clinical assessment and analysis of therapeutic progress.

There are 2 EMG/ETS custom programs for training purposes. All the parameters in the custom programs are adjustable by the therapist or by the individual user under professional guidance.

EMG parameters

  • Program duration: 99 minutes maximum
  • Threshold: 0.6 – 2000 µV; During Work period the user/patient is prompted to contract above threshold. In the Rest period the user / patient is prompted to relax the muscles.
  • Filter: Wide / Narrow
  • Biofeedback: Above / Below / Continue / OFF; Above the threshold; Below the threshold; Continue-sound; OFF – no bar graph sound.
  • Work / Rest time: 2 – 99 seconds
  • Threshold setting: Auto / Manual
  • Trial: Number of Work / Rest repetitions, 2 - 99

Training programs have patient mode and therapy mode. There are 2 custom EMG/ETS programs. All EMG Biofeedback settings can be changed/adjusted as required. Setting of custom programs should be done by trained medical professional or by the user under the guidance of the therapist.

ETS

EMG Triggered Stimulation (ETS) is a therapeutic fusion between EMG Biofeedback and Neuromuscular Stimulation. ETS is most commonly used in all types of muscle rehabilitation, including pelvic floor rehabilitation and pelvic floor muscles re-education. During ETS, the patient is required to contract the muscle as much as possible against target threshold. Once the target is achieved, the stimulation is triggered to supplement the naturally achieved muscle contraction beyond the contracting muscle’s actual ability.

ETS Threshold

Threshold is an EMG value measured in microvolts (µV). Stronger muscles which perform strong contractions have a higher threshold than weak or flaccid muscles. In ETS mode (EMG Triggered Stimulation), the user / patient needs to exceed to contract the muscles above the target threshold to trigger stimulation.

ETS treatment is especially useful for retraining and strengthening of the pelvic floor muscles. ETS mode also enables the user to stimulate weak muscles by setting a low level threshold target and adjusting the Work and Rest periods to individual patient needs.

Nu-Tek Levator Elite has an Automatic (pre-set) threshold for both EMG and ETS sessions – designed to adjust the EMG muscle strength scale and the point of ETS triggering to the actual level of the patient’s EMG biofeedback muscle contraction.

There is also a Manual threshold, which can be adjusted by the therapist as appropriate or required.

In the Patient mode, the Nu-Tek Levator Elite displays a simple and easy to understand EMG Biofeedback bar graph that illustrates pelvic floor muscle strength and helps the user to achieve their pre-set goals.

There are 6 levels on the Muscle Strength Grading Scale (Nu-Tek Scale) – measured in EMG microvolts (µV).

  1. Virtually no muscle contraction and very little microvolt reading.
  2. Slight muscle contraction with little movement, increased microvolt reading and a short contraction muscle holding time.
  3. Moderate muscle contraction with increased movement and microvolt reading and a longer muscle contraction holding time.
  4. Firm contraction and improved muscle holding time.
  5. Strong contraction with longer muscle holding time.
  6. Robust contraction with greatly improved muscle holding time.

Please note that the resting muscle tone has a value of 4 microvolts or lower.

ETS parameters

  • Program duration: 99 minutes maximum
  • Threshold: 0.6 – 2000 µV During Work period the user/patient is prompted to contract above threshold.
  • Trial: Number of Work / Rest repetitions, 2 – 99
  • STIM time: 1 – 99 seconds
  • Frequency: 2 – 100 Hz
  • Pulse width: 50 – 450 µS
  • Ramp up / Ramp down time: 0.1 – 9.9 seconds

Nu-Tek Levator Elite has a built-in capability to store and analyse the data and capture home compliance to prescribed treatment regime. Captured data are displayed on the LCD. Treatment statistics can be obtained after each complete session. After the session is completed, Nu-Tek Levator Elite displays the latest session statistics. Additionally, when used for training, the devices stores the statistics of EMG, ETS and STIM first training session statistics and then records every day training data accumulated per one calendar day. For EMG assessment, Levator Elite stores the weekly statistics by carrying out an assessment program weekly or fortnightly.

Nu-Tek Levator Elite can be linked to a PC using the optional Nu-Tek Software to record, store and analyse the measurements after each session. Periodically you will be able to download
